Report an Occurrence
All aircraft accidents and serious incidents must be notified to the Civil Aircraft Accident and Incident Investigation Unit (QAAI) by the most rapid practicable means available.
Notification may be made by contacting the QAAI 24-hour hotline or by submitting the online occurrence report form available on this website.

If in doubt, report the occurrence immediately.
The Duty Investigator will assess the information and determine whether a formal investigation is required.
When reporting, please include: aircraft type and registration, operator, flight details, location, number of occupants, nature of injuries, damage, and any dangerous goods on board.
Flight recorders (CVR/FDR) must be preserved and must not be reused or overwritten until formally released by QAAI.
What Should Not Be Reported to QAAI
The following occurrences should be reported to the Qatar Civil Aviation Authority (QCAA) under occurrence reporting regulations, but do not require direct notification to QAAI:
Routine technical defects or reliability issues with no safety impact
Minor day-to-day operational anomalies
PAN calls made solely for medical reasons or operational convenience (unless a safety risk was involved)
These events should continue to be managed within operators’ Safety Management Systems (SMS).
Remember: When in doubt, it is always better to report.
Every occurrence reported contributes to improving aviation safety in the State of Qatar.
